Usually, when a bottle is opened and it continuously foams without
being shaken it means that there is an infection of some kind. I'm not
sure where in your bottling process there might be a problem, but that
is my first thought. Or if somehow when you racked your beer onto the
priming solution, it didn't dissolve throughout the beer completely,
and caused some to be over carbed and others not.

My best suggestion? Kegging :)
